What medical records can I access in INTEGRIS&Me?

We share test results, provider notes, medication information, after visit summaries, and other types of health information with you through your INTEGRIS&Me account. Due to technical limitations and privacy laws, some information may not be readily available but may be requested through our Health Information Management department.

Navigate to the Health icon at the top of the INTEGRIS&Me Home page. Select Document Center and Request or Download Records. Click the hyperlink, send us a medical records request. The request form will open. Complete all applicable fields and submit the request.

Please note, this form should only be used if you are 18 years or older and to request your own medical records. If you need to request another person’s medical records, complete and submit the Patient Request for Health Information Form and provide supporting documentation if applicable. How do I request access to medical records I do not see in my INTEGRIS&Me portal account?

Navigate to the Health icon at the top of the INTEGRIS&Me Home page. Select Document Center and Request or Download Records. Click the hyperlink, send us a medical records request. The request form will open. Complete all applicable fields and submit the request.

How do I give another person access to my INTEGRIS&Me account?

If you are 18 or older, navigate to www.integrisandme.com and log into your account. Go to Health>Share My Record>Friends and Family Access. Click Invite Someone, complete the form, and click Send Invite.

If the person to whom you are granting access is not an INTEGRIS patient, or if you are not sure, please complete the Portal Proxy Request for Adult Form and an account will be created for them. The completed form may be emailed to HealthInfoManagement@integrisok.com, faxed to (405) 552-8773, or mailed to 3366 NW Expressway, Bld D. Ste. C-20, Oklahoma City, OK 73112.

How do I revoke my Proxy’s access to my portal?

If you are 18 or older, navigate to www.integrisandme.com and log into your account. Go to Health>Share My Record>Friends and Family Access. Select Revoke next to the person whose access you which to revoke.

How do I access the INTEGRIS&Me account of another person for whom I am the parent, guardian or legal representative?

If the patient is 18 years or older, complete and submit the Portal Proxy Request for Adult Form and provide supporting documents, as applicable, to show legal authority.

If the patient is a minor, complete and submit the Portal Proxy Request for Minor Child Form. If applicable, provide supporting documentation to show legal authority. Proxy access will expire when the patient reaches 18 years of age. In certain circumstances, the patient portal may not show certain medical information about a minor patient. Under state law 63 OK Stat § 63-2602, minor patients have the right to consent for certain services and that information must be kept confidential between the patient and provider. Information related to the prevention, diagnosis and treatment of any self-consenting minor who is or has been pregnant, afflicted with any reportable communicable disease, drug and substance abuse or abusive use of alcohol may not be accessible by the patient or the proxy.

How do I request a correction to an error in my medical record?

How do I update my address, phone, or other demographic information?

Navigate to www.integrisandme.com and log into your account. Go to the Profile Icon>Personal Information. Hover over the section you would like to edit and click EDIT.

How do I update my date of birth, name, or social security number?

Navigate to www.integrisandme.com and log into your account. Go to Messaging>Ask a Question>Customer Service Question, Choose Regarding: Chart Correction Request.

How do I print my test results and immunization records?

Test Results

1. Using your PC or mobile device internet browser, go to www.integrisandme.com and log into your INTEGRIS&Me account.
2. Navigate to Test Results and click on the test you would like to print.
3. Click the printer icon to generate a “printer friendly page”.
4. You may print, download and save, or forward this page.

Immunizations

1. Using your PC or mobile device internet browser, go to www.integrisandme.com and log into your INTEGRIS&Me account.
2. Navigate to Menu, then select Health Summary from the list of options.
3. Click on the Immunizations tab and select View Details or simply click on the immunization you would like to print.
4. Click the printer icon to generate a “printer friendly page”.
5. You may print, download and save, or forward this page.

**Print Note: On mobile device, internet browser must be used. Instructions do not apply to the INTEGRIS&Me mobile app.

How do I pay my bill?

Select the Billing icon at the top of the INTEGRIS&Me page and click Pay My Bill.

How do I ask a question about my bill or insurance?

Call INTEGRIS Customer Service at 405-252-8400 or toll free at 855-409-5458 or click Ask a Question from the Quick Links menu>Customer Service Question and select Billing from the drop-down list of topics in the message dialog. Additionally, you may click Pay My Bill and use our INTEGRIS Bill Pay portal to send a message to our Customer Service Center

How do I update my insurance information?

Timely filing of your insurance is very important to ensure all deadlines for billing your insurance plan are met. Whenever possible, provide your insurance information at the time of service. When that is not possible, please call INTEGRIS Customer Service, 405.252.8400, to provide insurance information. If the filing deadline has not been reached, we will bill your insurance plan right away.

How do I schedule an appointment?

Online scheduling is available for established patients through INTEGRIS&Me. From the Home page select Visits>Schedule an Appointment>Select the Provider>Reason for Visit>Select Date and Time>List Any Additional Information to be Addressed at the Visit>Schedule. If your provider does not show any available appointments, you will be presented with the option to request a preferred date and time for an appointment, and the clinic will contact you to schedule the appointment. Scheduling via INTEGRIS&Me is not available for all INTEGRIS providers. If your provider is not listed, please contact the clinic for more information on scheduling.

How do I see upcoming appointments?

From the INTEGRIS&Me portal Home page select Visits>Appointments and Visits to see all upcoming visits and all past visits. You can view the After-Visit Summary by selecting View After Visit Summary on past appointments.

Can I schedule an E-visit on the portal?

E-visits can be scheduled on the portal by logging into the Home page and choosing Visits>E-Visit>Accept the Terms and Conditions and enter the required information to schedule.

How do I send a message to my provider?

Go to the Messaging Icon>Ask A Question>New Medical Question and choose the provider you wish to send your message to through INTEGRIS&Me.

How do I request medical advice?

To request Medical Advice from your healthcare provider via the INTEGRIS&Me patient portal, please log in and visit the Home page. Go to the Messaging Icon>Ask A Question>New Medical Question and choose the provider you wish to send your message to through INTEGRIS&Me.

How do I request a medication refill?

To request a refill through the INTEGRIS&Me portal, log in and from the Home page, select Messaging. Select Request Rx Refill then choose the prescription that needs to be refilled and click the green Request Refill button.

How do I sign up?

Navigate to www.integrisandme.com and click Sign Up Now under New User. Enter the activation code, last 4 of social security number, and date of birth. If you do not have an activation code, click Sign Up Online and complete all fields of the electronic form. If the information entered is mismatched in our system, a member of the Health Information Management team will contact you for additional information or to provide further guidance.

Can I access INTEGRIS&Me via a mobile app?

You can access INTEGRIS & me from your Android or Apple phone by downloading the INTEGRIS&Me app from the Apple Store or Google Play. Search for INTEGRIS&Me and use your login and password to access your account. All features may not be available via the mobile app at this time - full access is available using the web or desktop version.
